This study examines an application that connects patients, physicians, nurses, and caregivers using symptom tracking, vital sign recording, medication reminders, appointment alerts, and secure communication. Data from wearable devices and home monitoring equipment can be uploaded for clinical review, helping healthcare teams identify worsening conditions and provide timely guidance. Personalized reminders encourage patients to follow prescribed treatment schedules, record missed doses, and request refills. Educational modules provide information on disease management, nutrition, physical activity, warning signs, and self-care practices. Dashboards allow clinicians to review trends, assess adherence, and adjust care plans when required. Data encryption, consent controls, and role-based access protect patient information.
